Need help hooking up my thermostat to run the circulating pump

I ran a new thermostat wire out to furnace to control the circulating pump when called for heat. Last year I had circulating pump running all the time because the thermostat wire was broke, Anyways cannot remember how to hook up the thermostat wire and the circulating pump to the switch in the attached picture.  Assume the item in the picture is a relay?

Is that a relay in the picture above? All I know is the item in the picture above is a switch. When the thermostat calls for heat it sends 24v to this switch which in turns complete the circuit so the circulating pump is energized and circulates the  hot water. My question is how is the item in the picture hooked up so it will work like I stated above?

That is a relay in the picture. Does the 24v power come from the thermostat?
The picture is not clear enough to see the connections. Can you get a close up of it?

I'd get a new 90-340 or 90-370 relay online and replace that one. It looks like it's seen better days. On the one in the picture, I think the lowest terminals are the coil, the upper rows are switches, but I can hardly see any markings so I might be wrong.

RSI - yes 24V from the thermostat.  I drew what I see on the relay see picture. I drew from memory at work.

E Yoder - yes the relay in the picture is old and has seen better days. It has taken me some time to figure out the issue was with the thermostat wire (broke/open) going out to the furnace so I just ran a new wire this last weekend. What I did last year was jumpered the pump to run non stop and in the process disconnected the wires from the relay and much time went by and forgot how it was hooked up.  Once I hook up again and confirm working/or not I will get a new relay.

If I am understanding correctly the bottom 2 blades are for the 24v coming from the thermostat. Once the tstat calls for heat the relay closes the top 2 to make the pump run?

I'm having a hard time seeing what you drew on my phone. But the new relay will show which are coil terminals for the 24 v and the normally open terminals are where you'd switch the 120 v pump power.

Sorry about the bad quality drawing but the top 2 blade connectors are normally open (on old relay), anyways took your recommendation and got a new relay so will hook up and try out this weekend. Thanks for the help!
