Pex through foundation leaking, best way to seal ?

I have the insulated pex pipe from Central Boiler,  i basicall made a whole it the cement foundation ran the CB pipe and electric conduit and sealer it with hydro cement I never had a issue last year but it started leaking today ( we got a ton of rain last night) I am thinking I am gonna have to dig a up the outside of the house were it comes it and try and put more hydro cement on it.

I always use spray foam in a can...  I use a whole can and let it mushroom outside all it wants

You may have to dig a pit outside under your pex pipe and then fill it was gravel to allow the water to sink below the level of the hole, maybe a couple feet deep would help
